Al Falah Center Membership Form

Please fill out this form to apply for Al Falah General Assembly Membership.

The fee is $100 per person per year.
You need to be a resident of the following NJ counties (Somerset, Morris, Middlesex, Mercer, Hunterdon, Union, Essex).
You need to be at least 18 years of age.
This form needs to be filled out by each member in the family who wishes to become a member and must read and accept the Al Falah Center Bylaws.
